Treatment notes

Write SOAP notes, sign them, and review chart history.

Treatment notes are clinic files. Clients never see them in the portal.

Open a note

Start from an appointment or from the client’s chart. Each note belongs to one visit and one client account.

Templates include Standard SOAP Notes. Owners can add more under My Clinic → Treatment Notes.

Save your signature

Everyone who signs notes keeps one signature, including practitioners.

  1. Select your avatar, then Profile.
  2. Under Signature (below Contact Number), choose Upload PNG or Draw signature.
  3. If you draw, use a mouse, finger, or stylus. Clear starts over. Save signature stores the drawing.
  4. The line under the preview shows when it was saved, for example Saved 24 Sep 2026. Remove deletes it.

You can keep only one image. Saving a new one replaces the current signature. It is saved as soon as you upload or draw. You do not need Update Account.

Clear a box

Each text box has a Clear button in its bottom-right corner. It empties that box only. To bring the text back, click in the box and press Ctrl+Z (Cmd+Z on Mac). Nothing is saved until you choose Save.

Translate a box to English

If your clinic owner has connected Google Gemini under Integrations, each text box (such as Subjective or Objective) has a Translate with AI button beside its label.

  1. Write the entry in any language, for example French, Tagalog, or Hindi. Mixing languages is fine.
  2. Click Translate with AI. The box is rewritten in clear English with the same formatting.
  3. Read the result before you save. To go back to your original text, click in the box and press Ctrl+Z (Cmd+Z on Mac).

The translation is not saved until you choose Save. Only the text of that box is sent to Google, not the client’s name. The button does not appear on signed notes or when Gemini is not connected.

Write and sign

Fill the note and choose Save. Then choose Sign & lock. If you still have unsaved changes, they are saved first. You need a signature on your profile before you can sign.

A signed note shows Signed and locked, with your name, the time, and the signature that was on your profile at that moment. The note fields cannot be edited.

To record a later change, write it under Addendum and choose Add addendum. Each addendum shows who wrote it and when. The signed note stays as it was.

Archive note hides a note from the chart, including a signed one. Archiving does not change the signed content. Archived notes stay in the clinic’s records and are logged in activity. Notes are never permanently deleted.

Chart history

On the client file, notes appear newest first. Search and filter by practitioner, date, or status. Print or export when you need a copy.
