Open Account to see the name, email, phone, and address the clinic has on file. You can upload a profile photo. The photo is visible to you and your clinic. It is not shown on a public page.

You cannot change name, email, phone, or address in the portal. Ask the clinic if something is wrong, or use Request a correction and describe what should change.

Request my records asks for a copy of this file. When the clinic marks it Ready, use Download PDF and Download JSON. You can have one open copy request and one open correction request at a time. A Refused request shows the clinic’s reason.

Use Sign out when you are done, especially on a shared device. Book an Appointment returns you to the public booking page for this clinic.
